최근 지역사회 통합돌봄이 노인복지의 주요한 이슈로 주목받고 있다. 이에 본 연구는 경기도 지역을 중심으로 노인의 재가노인돌봄서비스 이용의사를 살펴보고, 서비스 이용의사에 영향을 미치는 요인을 분석하기 위한 목적으로 수행되었다. 이를 위하여 2018년 경기도 지역주민복지욕구조사의 65세 이상 노인 9,811가구의 자료를 이용하였다. 재가노인돌봄서비스는 장기요양서비스와 노인맞춤돌봄서비스로 구분하여 이용의사를 종속변수로 설정하였고, 독립변수는 Anderson과 Newman(1973)의 서비스행동모델에 근거한 선행요인, 가능성요인, 욕구요인으로 구분하여 로지스틱회귀분석을 통해 재가노인돌봄서비스 이용의사에 미치는 영향을 분석하였다. 주요 결과는 다음과 같다. 첫째, 노인의 재가노인돌봄서비스 이용의사는 장기요양서비스 중 방문요양 이용의사가 54.4%로 가장 높았고, 방문간호 52.8%, 방문목욕 44.4% 등의 순으로 높게 나타났다. 노인맞춤돌봄서비스는 노인돌봄종합서비스 이용의사가 48.1%, 노인돌봄기본서비스 이용의사가 46.9%로 나타났다. 둘째, 재가노인돌봄서비스 이용의사 영향요인은 서비스유형에 따라 상이하게 나타났다. 서비스유형별 영향요인 결과를 종합하면, 선행요인의 ‘교육수준’, 가능성요인의 ‘소득계층’, 욕구요인의 ‘진단질환 수’, ‘일상생활유지’ 변인이 공통적으로 모든 장기요양서비스 및 노인맞춤돌봄서비스 이용의사에 영향을 미쳤다. 그 외 장기요양서비스이용의사에는 가능성요인 중 ‘서비스인지수준’ 및 ‘거주지역’ 변인이, 노인맞춤돌봄서비스 이용의사에는 가능성요인 중 ‘동거가족 수’, ‘주관적 경제수준’, ‘지역 내 자립도움정도’ 및 ‘의료시설 접근성’ 변인이 공통적으로 영향을 미친 것으로 나타났다. 연구 결과를 바탕으로 재가노인돌봄서비스의 보편적 서비스로의 확대, 재가노인돌봄서비스 내 건강관리 도모 프로그램 강화, 지역사회 환경적 특성을 고려한 노인돌봄계획 및 정책 수립의 필요성 강조 등의 정책적 시사점을 제시하였다. 본 연구는 지역사회 통합돌봄(Community Care)이 주요한 정책과제로 부각되는 시점에 노인돌봄서비스의 보편적인 확대를 위한 발전 방안을 Aging in Place 관점을 반영하여 논의했다는 점에서 의미가 있다.

This study aimed to estimate the elderly"s use intention of Community Care Service (CCS) and examined the influencing factors. For the analysis, it used 9,811 elderly cases from the data of the 2018 Community Residents Survey. The CCS variable was measured into the Long-Term Care Service (LCS) and Elderly Care Service (ECS). Moreover, the factors selected from the model of Anderson and Newman(1973), predisposing elements, enabling elements, and need elements, were used to analyze the effects on CCS"s use intention. The main results were as follows. First, the CCS use intention showed a different level in each service type. Among the LCS"s types, the willingness to use home-based care was highest with 54.4%, followed by visiting nursing 52.8%, visiting bathing 44.4%, and so on. In the ECS"s types, 48.1% of the elderly were willing to use care service for the elderly living alone, and 46.9% of the elderly were hoping to use care service for the blind zone of long-term care system. Second, the different factors were influencing the LCS and ECS use intention by service types. Notably, education, income class, number of illnesses, and difficulty in daily life variables significantly influenced all service intention types. In LCS, the service perception and region factors had a strong effect on service use intention than other variables. In ECS, family members" number, subjective economic status, neighborhood help for independence, and access to medical center factors significantly impacted on service use intention than other factors. Based on the results, the study suggested extending elderly care services for universal and enhancing the CCS system"s health program to protect the elders" declining health. Also, it presented the ways of activating community-centered care services for the elderly. The study had an implication of discussing CCS policy focused on aging in place perspective.
